Once you complete Railroad Quest: The Nuclear Option, speak with Doctor Carrington. It seems not all synths that teleported out of the Institute made it to safety. Carrington has a lead on one synth that’s in danger. After they’re saved, you have a care package that’ll help them get to safety. Then head to the indicated (random) location. All enemies are optional but better
off dead. Locate the synth, and speak with him or her.

Exit the area, and report in at Railroad HQ. You receive the following for your troubles. This quest is repeatable.
