After defeating Koala Kong, you gain access to Cortex Island. There are 10 levels here, plus a couple of extra levels. In addition, there are three bosses you must defeat. The levels are more dangerous and challenging. Several of the levels reward you with colored gems, which are used to unlock secret areas in previous levels where additional boxes are hidden.
