Find the south entrance to the cave and be ready to encounter an Elite Chieftain. Sneaking up behind him and using your Heavy Takedown skill is the easiest way to kill him and gain entry, but if you do not have that skill, lure him out and use your Two Handed Club, only performing heavy hits. Attacking with heavy hits keeps the Elite Chieftain off balance and prevents him from attacking or calling his allies.
With the Elite Chieftain dead, use your bow and arrow to pick off the Stationary Alarm and the two Spearmen nearby.
Carefully continue through the cave and perform a takedown on the Elite Scourge before using your bow to disable the second Stationary Alarm.
Sneak to the north side of the cave, leaving the two Villagers and Spearman alone while you work your way around into position to perform takedowns on each of the Elite Slingers that resides on the upper level. As you take down the second Elite Slinger you’ll be in position just in front of an Elite Scourge and Elite Chieftain. Using your beast here likely results in them being knocked down and killed.
Throw some rocks to attract the attention of the Elite Chieftain and Elite Slinger, separating them enough for you to perform a quick takedown. Sneak around to the Elite Chieftain and perform a takedown if possible, if you are unable to do this, use your spears to weaken the Elite Chieftain then finish him off with some arrows.
At this point you only have a Spearman to deal with, that you’ve left behind down by the Villagers. Follow the path back out toward the entrance you took to get into the cave. Turn back toward the Villagers for an easy shot on the Spearman. Once he is down the cave is clear, no need to kill the Villagers.
