Path of Exile 2 has several “optional” quests that are practically mandatory because they award you with passive skill points. If you have come across Act 3 in the campaign, you might have seen the Sacrificial Heart in your inventory but have no idea how to turn it in or what quest it is used for. Here’s a quick guide to the ritual altar location in which you must sacrifice the Sacrificial Heart in Path of Exile 2.
Where to Turn in the Sacrificial Heart Quest Item in Path of Exile 2
When you read the text or tooltip of the Sacrificial Heart, it mentions that you have to sacrifice it at the proper ritual site to give you two passive skill points, which is huge in this game. Luckily, the ritual site is located in the same area where the heart is found, Aggorat.
After exploring the Aggorat map, you’ll notice that the layout is designed with two main pathways or roads heading north. One of those pathways will lead you to the main quest, while the other leads to a ritual site where you sacrifice the Sacrificial Heart. Since the layouts are still technically different for each instance of the map, you’ll have to check both pathways to find the ritual site.
Once you have found the ritual site, you’ll see a Sacrificial Dagger next to it. Pick it up and stand on top of the altar to place the Sacrificial Heart. Once it is placed, you can now stab the heart with the dagger and finish the quest. This will instantly give you the passive skill points and you don’t have to talk to any other NPCs for it. Make sure to allocate your skill points carefully because respeccing skill points is expensive.

How to Get Back to Aggorat from Act 4 in Path of Exile 2
For players who blazed through the main quest since they found the path to finish Act 3 before going back to the ritual site in Aggorat, you are going to have a slight problem because the quest will take you to Act 4 on Cruel difficulty. You’ll go through the Riverbank again and fight the Bloated Miller boss.
Once you defeat the Bloated Miller boss, you’ll get to the Clearfell Encampment, where you can use the Waypoint to go back to Act 3. If you go back to Act 3, you’ll be in the “time skip” portion where you can go to Aggorat using the Waypoint.
However, if you are like me, who went to Act 3 and sold items to Oswald by going back through the gateway, you’ll have a problem going to that time skip.
The solution here is to go to the gateway in the Ziggurat Encampment and it will teleport you to the Riverbank again. Fight through the Bloated Miller boss again until you have access to the Waypoint in the town. Go back to Act 3 and you should be at the time skip now.
This could be a bug, and if so, I hope that Grinding Gear Games will provide a fix soon.
