The Harlequin Crest is one of those veteran items that has been around since the very beginning. And with the advent of Diablo 4, it’s made its grand return. Here’s how to get the Harlequin Crest in Diablo 4.
How to Get the Harlequin Crest in Diablo 4

Harlequin Crest has remained one of the most elusive Unique items around. But on June 24, Adam Jackson, the lead class designer for Diablo 4, confirmed several facts on how to obtain it.
- Harlequin Crest will only drop from enemies level 85+.
- It can drop from all the same places a normal Unique would drop.
- It will always drop at 820 power.
- It is very rare.
It’s theorized that it’s much more likely to drop from family-based mobs. For the Harlequin Crest, that means Cannibals and Cultists. The following Nightmare Dungeons are ideal for farming it:
- Conclave
- Cultist Refuge
- Dark Ravine
- Earthen Wound
- Guulrahn Slums
- Serpent’s Lair
- Shadowed Plunge
Unique items are some of the rarest and most powerful in the game, giving big stat boosts and even bigger buffs. But there’s none quite like the Harlequin Crest, which offers these potent stats:
- +1000 Maximum Life.
- 9.6% Cooldown Reduction [4.4-10.0]%.
- 11.5% Resource Generation [5.0-12.0]%.
- +20 All Stats +[20-28].
- Gain 20.0% [10.0-20]% Damage Reduction.
- In addition, gain +4 Ranks to all Skills.
But getting your hands on it is easier said than done. Unique Items are an extremely rare random drop that appears only in World Tier 3: Nightmare and World Tier 4: Torment. Because Diablo 4’s loot system is randomized, it’s possible to get a Unique Item from almost any source. That includes chests, enemy drops, Whispering Tree caches, and from the Purveyor of Curiosities.
Certain sources offer better chances, however. These sources are:
- Elite or Boss Enemies.
- Resplendent Chests found in Nightmare Dungeons.
- The Whispering Tree Caches (helmet).
- Helltide Chests (helmet).

I would also recommend that you farm out Events, as you’ll encounter plenty of Elite and Boss enemies while also getting Murmuring Obols. The Purveyor of Curiosities, located in most towns, will allow you to choose which armor piece you want. While your odds of getting the Harlequin Crest is low, you’ll at least be able to guarantee a helmet drop.
