People might be obsessed with a certain assassin’s headdress, but Andariel’s Visage is also extremely powerful, especially if you’re big into your poison builds. But Andariel’s Visage is one of the more elusive items in the game. Here’s how to get the unique item Andariel’s Visage in Diablo 4.

If you love doing tick damage and watching your enemies slowly corrode, Andariel’s Visage is the unique item for you. Not only does it poison your enemies, it offers pure stat boosts across the board, increases your attack speed by nearly 10%, and boosts your Lucky Hit up to 19%. It’s a good item, is what I’m saying. But it’s also one of the rarest in the game.

Andariel’s Visage is one of six Unique items that fall into the “ultra rare” category. Here’s what we know about it:

  • Andariel’s Visage can drop from any level 85+ enemy.
  • You can get it in all the same places you’d find a normal unique item, including Helltide Chests and The Tree of Whisper caches.
  • Its power level is set to a default of 820.

This information was confirmed by lead class designer Adam Jackson on Twitter.

That means one of the most consistent ways to find it at lower levels is to use a Level 31 Nightmare Sigil. It’s also theorized that certain families of monsters (such as beasts, vampires, cultists) have a higher tendency to drop certain categories of items.

For Andariel’s Visage, the best family of monsters to farm are Cannibals and Cultists, which make the following Nightmare Dungeons ideal:

  • Conclave
  • Cultist Refuge
  • Dark Ravine
  • Earthen Wound
  • Guulrahn Slums
  • Serpent’s Lair
  • Shadowed Plunge
