In Back 4 Blood, players start out with four character options called Cleaners including Walker, Mom, Evangelo, and Holly. While these four characters are all excellent in their own right, there are four additional characters that you can unlock in Back 4 Blood which include Doc, Hoffman, Jim, and Karlee.

Back 4 Blood | How to Unlock All Characters, Cleaners
The quick answer to the question of how to unlock all characters in Back 4 Blood is that you need to play all of Act 1 online with other players. You can’t unlock the four additional Cleaners in Back 4 Blood in the solo campaign – at least not at the time of this writing.
Note that in regards to playing all of Act 1, this means completing all four levels featured in Act 1 which include Resurgence, Tunnel of Blood, Pain Train, and The Crossing. Once you’ve worked your way through to the end of The Crossing, you should see a cutscene with the four additional Cleaners that you can unlock.
Completing Act 1 (online, not in the solo campaign) will also get you the Paid The Toll achievement along with 150 Supply Points. Wondering what the stats are for the four additional Cleaners you can unlock in Back 4 Blood as well as the four original Cleaners you’re given access to?

Again, you’re given access to the first four Cleaners (Walker, Holly, Mom, Evangelo) right at the beginning of the game without you needing to do anything.
To unlock the additional four Cleaners, you will need to play all of Act 1 online with other players. You can’t unlock these four additional Cleaners in the solo campaign.
By completing all four levels of Act 1 online, you’ll unlock Cleaners including Doc, Karlee, Jim, and Hoffman. We hope this helps!
