Diablo 4 players can get Baneful Hearts by defeating Hellborn monsters and opening Tortured Gifts of Mysteries. Both the elite demons and the Tortured Gifts of Mysteries chests can be found during Helltide events.

The Hellborn enemies are new to Season 4 and must be triggered before they will show up. There is a new Threat meter next to your minimap that fills up by defeating enemies and opening Tortured chests, and filling it up three levels will spawn a Hellborn enemy. Defeating one can be rewarded with a Baneful Heart

The other option is to open Tortured Gifts of Mysteries chests in Helltide regions. Each chest costs 100 Aberrant Cinders, earned by defeating normal enemies in Helltide, and is marked on the map when nearby. This is the best way to earn Baneful Hearts as you can gain hundreds of Aberrant Cinders during one Helltide session.

Baneful Hearts are used to open up Accursed Ritual sites located in Helltide regions. These are similar to the Blood Harvest towers back in Season 2 and are available in any of the four World Tiers.

Any player can place three Baneful Hearts at the Accursed Ritual to spawn a few waves of enemies and end with the powerful Blood Maiden boss. Defeating the elite demon will drop a ton of loot. However, players who deposit at least one Baneful Heart will receive more drops than players who simply participated in the event.
