Diablo 4 is full of unique items that are specifically made for a single class, or can be found universally as a great way to make your build more powerful on any class. Only six of those items are considered Ultra Rare Uniques though, and in this guide, I will cover all of their effects. Here are all Ultra Rare Uniques in Diablo 4.
Diablo 4 – All Ultra Rare Uniques and Effects
There are six Ultra Rare Unique items in Diablo 4, and to earn them, you must complete content that is level 85 or higher. Any monsters that are below that level will have no chance of dropping one of these. But before you start farming for one of the six major items, I recommend checking out the full list of effects.

Doombringer | One-Handed Sword | Lucky Hit: Up to a [15 – 25%] chance to deal 5,822 Shadow damage to surrounding enemies and reduce their damage done by 20% for 5 seconds. | |
Andariel’s Visage | Helm | Lucky Hit: Up to a [15 – 20%] chance to trigger a poison nova that applies 10,077 Poisoning damage over 5 seconds to enemies in the area. | |
Ring of Starless Skies | Ring | Each consecutive Core Skill cast reduces the Resource cost of your next Core Skill by [8 – 12%] up to a maximum of 40%. |
Keep in mind when you look for these items that they are called Ultra Rare Uniques for a reason. Players in Diablo 4 have been searching nonstop for the items and nearly the entire player base has had no luck. They are so rare that some players have even wondered if they exist until Blizzard confirmed it. This is just another reason for me to grind out those higher levels.
