BG3’s fish people, who are officially called Kuo-Toa, are located in the Underdark in Act 1. They’re in a location that is easy to miss, but I have instructions to get you right to their front door.

From the Underdark – Selunite Outpost, jump through the window on the rock ledge heading west. Continue down, defeat the Specter, and continue down like you are going to the Arcane Tower.

Climb down the last Cragged Rock stairs so you are on the red mushroom level. Turn south and you’ll see a few large platform mushrooms you can jump onto, along with a Cragged Rock going down. This area is very dark, making it easy to miss.

Heading down the Cragged Rock takes you to The Festering Cove, the home of our new fish friends. Make sure to save before entering so you can try out different outcomes.

Upon approaching the Kuo-Toa, the fish people in BG3, Pooldripp the Zealous will tell you that they are worshipping their god named BOOOAL. You can ask questions to figure out what is going on here and who BOOOAL is, but if you want to be worshipped by the fish people, you need to choose the following options:

  • [Investigation] Get your bearings. What have you stumbled in on?
  • [Appearance] Red caps love violence. Bait him with an exaggerated story about bloody fights.
  • I’ll just kill you and claim it for myself.

In my playthrough, I didn’t roll well enough to convince the fish people that BOOOAL was a fraud and I was their real god. Instead, I convinced BOOOAL to not kill me because I could get more blood for him. He gave me the Sickle of BOOOAL which does 2d3+3 slashing damage, has Lacerate, and has BOOOAL’s Blessing giving the wielder Advantage on Attack Rolls against Bleeding creatures. But like everything in BG3, this dialogue has more than one way of ending.

If you’re looking for a different outcome or reward than the option already suggested, you’re in luck. A lot of these options have dice rolls, so make sure that Inspiration is stocked up in case you fail. As stated above, you can get out of this dialogue peacefully if you decide to murder other nameless faces for the false god. But what if you want to give him immediate bloodshed?

All outcomes with fish people and BOOOAL in Baldur’s Gate 3 (BG3)

Sacrifice Origin Companion

You have the option to sacrifice one of your Origin companions such as Karlach or Wyll which kills them permanently in exchange for BOOOAL’s Blessing. With the blessing, as long as there are fish people to worship the false god, you have Advantage on Attack Rolls against Bleeding creatures no matter what weapon you use.

If you can’t convince the fish people their god is fake, and you don’t want to sacrifice a party member or promise them to spill blood in their name somewhere else, a fight with BOOOAL and all of his fish people will ensue. This route allows you to still grab the Sickle off of Pooldripp the Zealous’ body, so at least it wasn’t all for naught.

Bard-only option

Finally, there is a secret option available only to Bards. If your character is a Bard, you have the option to play a little tune that turns all of BOOOAL’s followers on him. You’ll first have to pass a DC 15 check to remember the history of the fish people, allowing you to roll again, this time for a DC 10 to spin the tale of their false idol.

After their minds are free from the Redcap’s control, you have the option of telling the fish people the truth, giving them a new god to worship, or helping the Redcap continue their facade. If you want to keep going the route of the performing Bard, you’ll have to succeed in one final dice roll with a DC 20 in order to get the fish people to help you kill BOOOAL.

As is the case with all dialogue options in Baldur’s Gate 3, there are multiple outcomes. Play your way or trick the fish people into believing you are their god. Either way, you’ll probably want to discover this extremely rare Greatsword while you’re still in Act 1 or learn the origin of Gortash’s plan in Act 3.
