Besides the companion approval ratings, leaving the Mayor to his fate will give you the “A Growing Corruption” side quest later on in Dragon Age: The Veilguard. This is a completely missable side quest and will present itself in the game if you choose to leave the Mayor to his fate.
Essentially, the blight takes over the Mayor if you leave him to die while the blighted vine traps him. In “A Growing Corruption” side quest, you will be tasked to defeat Mayor Julius, who has become a powerful blighted boss and must be stopped.

Saving the Mayor in Dragon Age: The Vielguard will increase the player’s approval with Neve. However, Harding and Bellara’s approval of the Rook will decrease.
While choosing to save the Mayor spares him from the blight, he ultimately returns to his old ways. During the “Blood of Arlathan” quest, you will discover that the Mayor has once again turned to the dark side—this time reappearing in the Temple of the Sun as a member of the Venatori.
Third Option: Banish the Mayor
If you happen to have picked the Grey Wardens faction at the start of Dragon Age: The Veilguard, during save or leave Mayor to his fate choice, you will get another option. The third choice involves banishing the Mayor to the Grey Wardens.
This action will decrease Rook’s approval with Harding and Bellara, but it will greatly increase approval with Neve. So, if you care less about what Harding and Bellara think about you and want to romance Neve as quickly as possible, go with this optional choice, provided you are one of the Grey Wardens faction, of course.
