The Heart of Mystery in Pathfinder: Wrath of the Righteous is actually west of Conundrum Unsolved. However, looking at the world map shows a ridge that prevents you from traveling there directly.

 

Instead, you’ll need to start from Drezen. From there, keep going south-southwest, past Wintersun. Continue along the twisting roads until the map turns purple, signifying that you’ve reached the Wounded Lands region. Eventually, you’ll reach the Heart of Mystery which is close to Terendelev’s Lair (a place you’ll visit to unlock the Gold Dragon Mythic Path).

Once inside the Heart of Mystery, the Ancient Ghost will greet you and tell you about your next objective in the Secrets of Creation quest. In this chamber, you’ll see four portals leading to four different locations. Each destination has its own puzzle to solve. Once you’re done solving all of these, that’s the only time you’ll unlock the last trial.

There’s also a circuitous route that you could stumble upon if you meet the Ancient Ghost in a random encounter. He’ll give you the location of the other caves, as well as several scrolls if you pass a Knowledge (Arcana) skill check.

Find these destinations on the world map and complete the brain teasers therein: Core of the Riddle, Forgotten Secrets, Legacy of the Ancients, and Final Veil.
